Health Contract MUST be printed out, initialed where applicable, and signed by all Adoptive Families, and must
accompany your Security Deposit to hold your puppy.
HEALTH
STATEMENT:
Our Sires & Dams are in EXCELLENT
HEALTH. They receive the best of care from the moment they become OURS. We protect our dogs against heartworms, fleas/ticks, intestinal worms and parasites,
rabies, and other diseases as directed by our veterinarian. Your puppy is
as HEALTHY as we can possibly have him/her when he/she leaves our home. Your puppy will have received his/her
first set of immunizations and will require the next set approximately 3-to-4 weeks from
Adoption Day. Our 72-hour Health Guarantee encourages
you to visit your veterinarian for a “well-baby” exam. REMEMBER TO TAKE A FRESH PUPPY STOOL SAMPLE WITH
YOU! (Request a stool check at this exam!) While there, PLEASE discuss/purchase your puppy's monthly heartworm
prevention program, and monthly flea prevention products. [We
use oral, Ivermectin-Based monthly heartworm prevention, AND monthly-applied "Frontline Plus" for
flea/tick prevention.] Our
dogs are fed HIGH QUALITY FOODS with NO CORN (corn is non-digestible and only produces more poop),
NO WHEAT (wheat produces yeast on the skin, which increases the potential for skin issues), and NO
SOY (not your worst enemy, but not your best friend, either!).
OUR PUPPIES ARE SOLD AS FAMILY PETS. We do NOT sell puppies
for breeding purposes. We recommend spay/neuter at age 5 to 6 months, depending on the
minimum age set by your veterinarian. Spay/Neuter promotes a HAPPY, HEALTHY pet!

Your Puppy’s
Health Care includes the following:
- Veterinarian Tail Dock (Screw-tails may remain intact).
- Wormed on Schedule, beginning at age 2 weeks; Treated for any intestinal parasites/worms
as needed.
- 1st Set of Immunizations (Distemper/Parvo shot).
- Lots of TLC; Well-adjusted to the human touch!
- Bath on Adoption Day.
You will be given a "Few Days' Worth" zip-lock baggie of food that your puppy has been eating
for weeks prior to adoption. We strongly suggest HIGH QUALITY food that contains NO CORN, NO WHEAT and
NO SOY. Your puppy has been raised on "BLUE BUFFALO" dry food, (purchased at PetSmart). You will receive
details on feeding your puppy on or before adoption day. The price
of high-quality food is far less than the price of unnecessary visits to your vet due to poor nutrition. Your
dog's diet directly effects the skin, coat, energy levels, inner organs and overall HEALTH of your dog.

PLEASE PROTECT YOUR NEW BABY:
UNTIL YOUR PUPPY HAS RECEIVED ALL PUPPY SHOTS, HE/SHE IS AT RISK FOR CONTRACTING
DISEASES! Distemper, Parvo, and other common diseases are potentially fatal to your new puppy. PLEASE
do NOT expose your puppy to animals other than your own family pets, and ONLY if they are properly immunized
/ up-to-date. As tempting as it will be to show off your new bulldogge at the local pet-supply store or
at your local park, these places will just about guarantee a trip to the veterinarian. Not all
pet owners immunize as they should. Your puppy can catch a deadly disease in a hurry, even from human contact
with a sick animal. Beware: Human SHOES/feet AND Animal paws can carry
many diseases. CONSULT WITH YOUR VETERINARIAN regarding the health and safety of your new puppy. There
will be plenty of time to show-off your "new baby" once he/she has received all puppy shots & the rabies vaccine.
WE HIGHLY RECOMMEND Obedience Classes AFTER your puppy has received all 3 sets of puppy shots, which should be around 12 to
13 weeks of age.

HEALTH GUARANTEE:
We WILL
NOT breed a dog known to have health problems! We
welcome you to contact our veterinarian for reference information about our breeding practices and to confirm that we take
excellent care of our dogs. We follow a strict regimen of preventive maintenance on all of our dogs and puppies. It
is our hope that you will follow the advice and instruction of your veterinarian, beginning immediately upon the
adoption of your puppy.
You have 72 HOURS (including
Adoption Day) to have your puppy examined by your veterinarian. IF
your puppy is diagnosed with a heart, lung, liver or bone malfunction within the first 72 hours of adoption, we will refund
your adoption fee upon immediate return of the puppy and veterinarian documentation. Your puppy must
be described in detail (sex, color/markings, etc.) on the veterinarian's documentation. This health guarantee does NOT
cover intestinal worms or parasites, mites, skin or eye-related conditions, or "outie" belly-buttons. We
will NOT refund veterinary, travel, or shipping expenses. This health guarantee
expires seventy-two (72) hours after Adoption.

If shipping your puppy via flight: DELTA
is the only airline which allows puppies/dogs to be shipped out of the Myrtle Beach Airport (MYR). Airlines require a veterinarian's Health Certificate. Your puppy will be examined by MY veterinarian within 10 days of scheduled flight date. Adoptive
Family is financially responsible for flight ticket, Health Certificate veterinary visit, extra immunizations
if age-appropriate, carrier/crate required for flight, and any travel expenses (mileage/hotel/meals) involved. During Summer months, we cannot ship puppies by plane, but we DO have
a flight-transport service which offers your puppy one-on-one care; your puppy flies in the cabin with the transporter.
